Chronic Care Specialists

EliaCore is led by a team of physicians and mid-level providers. All care plans are initiated by our providers. Our providers collaborate with YOUR primary provider with ongoing and urgent updates as needed.

Our Behavioral Healthcare Managers have specialized training in Psychology and/or Behavioral Health, They work with our patients face to face in partnered facilities to promote positive mental, social and behavioral well-being.

Our Chronic Care Specialists work from home and on-site at our partnered facilities. These specialists work with the patient and/or caregiver to provide collaboration and coordination with primary care, pharmacy, hospital and all specialty care involved.

Remote Patient Monitoring Specialists

Our RPM Specialists are clinicians dedicated to collecting physiological data that is electronically captured in our health record system in near real-time. This enables our team to observe patients more closely, gather necessary data, and make adjustments to improve care outcomes.

Concierge Care Specialists

Our Concierge Care Specialists are here to support you and your family throughout this journey. They provide coordinated care between the entire healthcare team as well as provide support with navigating facility transition. From start to finish, they will assist with finding appropriate short and long-term accommodations to best suit your needs.

Readmission Prevention Specialists

Our Readmission Prevention team aims to provide at-risk patients with world-class clinical care to greatly decrease the likelihood of returning to the hospital. These medical providers are on-site at our partnered facilities conducting preventative assessments and collaborating with the facility care team and family.
